Permission consent

Helps us to keep everyone safe and plan for a positive experience

Before a child or young person under 18 joins an activity, event or program at the Salvos we ask their parent or guardian to complete a permission form.

Why we collect this information

The details you provide help our leaders: 

  • Respond well in an emergency
  • Make sure activities are safe and enjoyable
  • Support the needs of all participants.

All information is stored securely and used according to our Privacy Policy. 

Talking about the form 

We encourage parents and guardians to talk with their child or young person about the behaviours expected during the program. You can choose how much information to share - but the more we know, the better we can support your child. 

Expected participant behaviours 

Children and young people agree to: 

  • Respect privacy - No photos or screenshots without permission
  • Be fair and kind - Treat everyone equally and listen to others
  • Speak and act respectfully - Help everyone have fun
  • Follow safety directions - Listen to leaders and assistants 

What we ask parents and guardians 

We will request permission for: 

  • Participation in the activity or program
  • Transport to off-site activities
  • Medical care and emergency management
  • Photographs and videos
  • Feedback forms 

You will also be asked to acknowledge:

  • Our Privacy Policy
  • Acceptance of risk outside duty of care
  • That participation is voluntary and permission can be withdrawn
  • That Christian beliefs may be reflected in activities
